The animal care team at Connecticut Veterinary Center is pleased to welcome you to our animal hospitals. We offer a variety of electronic forms to make your visit easier for you and your pet. These forms can be completed before your arrival, saving you time and stress when you bring your pet to our animal hospitals.

First Visits to Our Animal Hospitals

If you’re coming in for a first visit to one of our locations, please complete the “Let’s Get Acquainted” form in advance of your appointment so that we can get to know you and your pet.

Boarding Visits at Our Animal Hospitals

If your pet is scheduled for a boarding visit at one of our animal hospitals, complete the “Boarding Form” before your arrival. This gives us all the information we need to provide the best, most complete veterinary care to your best friend.
